RESERVATIONS AND PAYMENTS

Reservations: Reservation fee of Php1,000 is optional but assures the participant of accommodation at the venue and a seat to the chartered bus. Registration fee for:

1. Live-in Participants are entitled to accommodation for 3days/2nights from April 29at 10:00am (check-in) until May 1 at 12:00 noon (check-out), seminar meals (am & pm snacks, lunches and dinner), breakfast on April 30 and May 1, seminar kits, handouts, certificates, and the cultural tour.

2. Live-out Participants are entitled to seminar meals (am & pm snacks, lunches and dinner on April 29 only), seminar kits, handouts, and certificates.

3. Accompanying Persons are entitled to all seminar meals, accommodation and the cultural tour only.

Important Note: Participants may come and check-in ahead of the seminar date and time at their own expenses.

Cash or Institutional Check Payments

Note: Participants must bear the bank charges themselves. Personal checks are not accepted. Please deposit exact amount of registration/reservation fees to the following account: Philippine Association of Academic and Research Librarians or “PAARL”

Please attach a copy of your deposit receipt with this form and send or fax it to any of the contact persons.
